Main duties of the job We are looking for a passionate, committed band 6 midwife toprovide maternity care within our community midwifery team based at BishopAuckland.
You will be responsible for the planning, implementation andevaluation of individualised care plans which ensure delivery of holistic,research based care, prioritising communication, informed consent andrelationship building.
CDDFT community midwives are passionate aboutfacilitating antenatal and postnatal continuity of care and you will be supportedas an autonomous practitioner to plan your diary around your caseload, ensuringthat women and families are at the centre of everything we do.In return, we offer a personal induction programme where youwill be supported by our practice development team to continue to develop yourskills and thrive in your new role.

Our team at Bishop Auckland provide care towomen and families across a varied setting, including the beautiful DurhamDales .
Our community hospital works closely with our maternity units at the UniversityHospital North Durham and at Darlington Memorial.

The focusof our dedicated community teams is patient centered and individualised care,providing support and continuity throughout the antenatal and postnatal periodand promoting choice to families from across our diverse communities.
We are looking for an individual with a passion for healthpromotion, as the role of community midwife at our trust includes workingwithin the UNICEF framework for Infant Feeding, and close working relationshipswith our specialist antenatal and newborn screening Team to deliver allelements of the National Screening programme.
You will be very well supported with a bespoke induction andtraining package by members of our specialist Practice Development Team whowill ensure your support as you transition to your new role both professionallyand personally.

Each team also has access to admin support andband 3 Maternity Support Workers who offering close support to each team.
Youwill also be able to access personalised pastoral care via our specialistworkforce experience teams, including our retention lead midwife and the PMAservice.
