We are pleased to share this opportunity for a motivated and enthusiastic mental health professional to join the Chester Community Mental Health Team.
(Qualified Nurse, Occupational Therapist, Social worker etc).
The successful candidate would be joining an established, knowledgeable and enthusiastic team who support each other and strive to deliver an exceptional service for its patients.

We have robust systems in place to ensure that care can be delivered safely for patients, staff and the wider organisation, utilising a whole team approach to care.
All team members support each other in the effective management and delivery of services, within this role you would experience a wide variety of duties, experiences and challenges making each day different, exhilarating and feeling a sense of real achievement.
This role would primarily involve key-working a varied yet manageable case load.
Main duties of the job This is a great time to be joining the team following a period of investment in training and resources.
The post holder will work within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness.
The post holder will key-work a caseload which will involve ongoing assessment, formulation, care planning, risk assessments, completing reviews and offering tailored interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies where necessary.
Excellent communication, report writing, and assessment skills are essential for this post.
Applicants should have the appropriate qualifications, a comprehensive working knowledge of mental health disorders and of the Mental Health Act.
