(CMHS) You will be joining the CMHS at anexciting point of change influencing how Community Mental Health Care isdelivered leading local services in providing outstanding care in the communityfor clients.
Main duties of the job The Core Team will work closely with other services to provide specialist mental health assessment and evidenced based treatments for people with severe and enduring mental health difficulties and complex presentations.

Job description Job responsibilities Thepost-holder is responsible to the Manager of the Community Mental HealthService (CMHS) for the assessment of needs, development, implementation andevaluation of care for patients with a diagnosis of a severe and enduringmental health diagnosis.Thepractitioner is responsible for assessment of needs, development,implementation and evaluation of care, within the Personalised Care Framework.Thepost holder may be required to supervise junior staff within the CMHS.Accountability:Thepost holder is accountable to the Community Services Manager through the Line Managementsystem within the CMHS.Pay & Conditions:Thepost holder must have completed training as a first level registered nurse inmental health with a currently valid registration with the Nursing andMidwifery Council.Salarywill be Band 5.KeyTasks and Responsibilities of the Post:Thepost holder will assess all new referrals as appropriate, at home or in aclinic setting, in accordance with the agreed Operational Policy.Thepost holder will provide nursing care and other interventions as appropriate inthe right setting, in accordance with the agreed Operational Policy.Thepost holder will arrange for a defined group of practice patients to accessother parts of Adult Mental Health Services and work closely with all otherclinical professionals Consultant Psychiatrist, Clinical Psychologist,Social Worker etc.Thepost holder will act as a Key Worker for a defined group of patients andorganise care in accordance with the Personalised Care Framework.Thepost-holder will take part in clinical audits of the CMHS.
Thepost holder will ensure that their clinical practice is based on evidence andgood practice and in accordance with NICE guidance, and Livewell Southwestpolicy and procedures.Thepost holder will be individually accountable for their practice in accordancewith the NMC professional code of conduct.Staff Management: Thepost holder may be responsible for supervising some junior staff within theCMHS.
Liaison andAdministration: Thepost holder will maintain close links with in-patient services at Glenbourne,Home Treatment Team, Social Care, Primary Care services, MAIS, and thevoluntary sector.
Thepost holder will keep up to date electronic/paper records of all patient andstaff activity, ensuring these are kept in a safe place, in accordance with NMCguidelines, and Livewell Southwest policies including Data Protection Act andthe Mental Health Act.
The post holder will be aware of NMC and LivewellSouthwest guidelines on record keeping, confidentiality, Mental Health Act andData Protection Act.
Professional Developmentand Education: Thepost holder will be required to keep up-to-date this will include Mandatorytraining, developments in mental health nursing or sphere of practice, and NMCRevalidation.
Thepost holder will participate in the provision of relevant community placementsand training programmes for nurses.
Thepost holder will be required to ensure that they receive appropriate regularline management and clinical supervision in line with current policies of LivewellSouthwest.

Experience Essential Relevant experience of working with adults with a history of severe and enduring mental illness.
