Job summary We are currently looking for a CPN Band 6 for our Adult Community Mental Health Services at Oxford House.
The post holder will work in close collaboration with other professionals, including the wider multi - in assessing service user's needs, devising care plans and evaluating outcomes that promote service user empowerment and recovery.
The post holder, with the MDT will provide high quality, individualized and evidence based care and treatment, which addresses the needs of the service users and their families and meets individual specialist health needs.
Main duties of the job Be responsible for the assessment, planning, implementation and evaluation of high quality care and treatment for those service users that are under Adult Community Mental Health Services Undertake training in phlebotomy in order to monitor the physical health of people Manage high levels of risk in the community in partnership with the consultant psychiatrists and senior support workers, promoting service user recovery as well as safety whenever possible Doing First Contact Assessment on weekly basis and Duty Having a case load that is appropriate for a Band 6 nurse Be responsible for liaising with the GP practices in the quadrant for which the nurse is responsible.
Be required to teach other nursing and non-nursing personnel.
Be a role model through their own good practice.
Be responsible for maintaining clinical standards, and team systemsunder the direction of the Team Leader and Professional Lead.
Assist the team in maintaining effective team working processes, including outcome measures.

Person Specification QUALIFICATIONS/EDUCATION/TRAINING Essential RMN or Dip/Bsc (Hons) in Mental Health Nursing ENB 998/Preceptorship and Mentorship Desirable ENB812/Bsc in Specialist Community Nursing Practice (Mental Health) Psychosocial interventions PREVIOUS EXPERIENCE Essential Experience of working in a range of settings relevant to the post Extensive post registration experience with an experience at Band 5 Experience of working in acute mental health inpatient/day care services Experience of working in situations of crisis and emotionally demanding environments Experience and knowledge of safeguarding process Experience of teaching and training Experience and knowledge of working in a multi-ethnic community Experience of delivering evidence based care Desirable Experience of working with acute mental health problems in a community setting
