Job summary The Community Learning Disability Team has a vacancy for a Band 6 Nurse.
This role will support people with a learning disability who are placed out of area, and work with people in the community who require nurse input.
The Learning Disability Team is a multi-disciplinary team who are passionate about providing the best possible care.

Main duties of the job The post holder will be responsible, together with the wider team, for providing care, interventions and health promotion to people with learning disabilities.
*It is essential that robust communication networks with clients, carers and other health workers and care agencies are promoted.
*Utilise a range of communication skills to ensure that clients with learning disability understand the implications of their health issues, including complex health problems.
*Utilise a range of communication skills to assist capacity for consent to treatment and provide advice regarding best interests when a client lacks capacity.
*Maintain a knowledge / skills base and evidence the same through a personal professional portfolio and act in accordance with the NMC code of professional conduct.The post holder will also undertake a supervisory role in respect to multi-disciplinary junior team members and work alongside the Team Leader on clinical issues such as therapeutic approaches and care co-ordination.This role will lead on supporting people with a Learning Disability who have been placed out of area.

Person Specification Qualifications Essential *RNLD or RNMH with live NMC registration *Evidence of on-going development and post registration training *Evidence of ongoing professional development related to Learning Disabilities *Demonstrable Care Co-Ordination skills *Training in Risk assessment and management skills *Experience in the assessment and management of people with moderate / severe Learning Disabilities who present with co-morbidities *Evidence of undertaking audit work *Innovative, creative and a willingness to try new ideas Desirable *Completion of further clinical training *Completion of leadership training Experience Essential *Self- motivated in professional and service development *Attention to personal detail & presentation *Awareness of personal safety *Organisational skills *Demonstrate evidence based practice *Excellent communication skills and ability to adapt communication styles *Ability to write and read reports *Confident with IT *Display the ability to effectively manage the caseload, prioritise and delegated work *Ability to work within a MDT ensure policies/ procedures are followed *Work effectively in difficult circumstances and seek support when needed *Demonstrate effective use of resources *Be flexible/ Good interpersonal skills *Can demonstrate the Trust Values and expected behaviours Desirable *Quality service improvement *Developing pathways and clinical policies *Supporting a student on placement *Delivering training *Support with team development *Understanding of the process of involvement with people with a learning disability who are placed off the Isle of Wight
