Job summary We are looking for an enthusiastic, highly motivated, dynamic and dedicated Learning Disability Nurse to join our established innovative, community-based Learning Disability team in East Cheshire.Dependent upon experience, the successful applicant will either commence as a Band 5 and progress to a band 6, or commence at Band 6, please speak with the team manager for more information regarding this.You will be responsible for delivering high-quality clinical care and ensuring all work is needs led, evidence based and in line with current legislation.You will also support the development of services in line with the workstreams that are the focus for the care group, which includes, mental health, forensic, physical health, neurodevelopmental conditions and cognitive impairment.If you are passionate about making a difference to the lives of adults with a learning disability, and those who care for them and want to contribute to an organisation which is continuously striving to improve then we would love to hear from you.For newly qualified nurses (band 5), there will be the opportunity to support with preceptorship.There are two bases used by this team, one being in Macclesfield (Rosemount) and one being in Crewe (Catherine House), and the location of the base will be discussed with the successful applicant.
Main duties of the job The post holder will carry a caseload of clients with learning disabilities who have a varied level of complex needs, to deliver care that will reduce health inequalities for people including behaviours of concern and/or autism.Lead on meetings, including care planning, and training around people on the caseload to make sure they are receiving the best support for them.Depending on experience and level when coming into the team, there would be an expectation in terms of being responsible for providing clinical supervision and appraisals of nursing staff within the team.To supervise students while on placement with the team.CWP hosts CANDDID, the Centre for Autism Neurodevelopmental disorders and Intellectual Disabilities, which carries out research, education and training, and all clinicians are encouraged and supported to develop research and QI skills.

Person Specification Qualifications Essential First level nurse.
RNLD/RNMH/ RMN (degree level).
Evidence of post registration module training in specialist area to diploma/degree level physical health in learning disabilities module; epilepsy module; dementia module Evidence of post registration training in - autism - epilepsy Demonstrate commitment to ongoing learning & development.
Desirable Nurse prescribing qualification.
Knowledge Essential Ability to assess/Plan/Evaluate care to clients and carers.
Ability to participate in and develop nursing practice Support clients in crisis Excellent record keeping Ability to chair meetings and lead a team CPA Deliver training to other professionals and services.
Special interests related to current service developments ( epilepsy; autism etc).
Experience Essential A reasonable amount of post qualifying experience of working with adults with a Learning Disability in a community setting Direct experience of working with clients with complex needs challenging behaviour, epilepsy, mental health problems, continuing health care Clinical supervision of staff.
Delivery of training to other staff, service users and carers Positive attitude to people with LD Awareness of current issues.
Awareness of relevant legislation and good practice relating to learning disabilities (Valuing People; Mental Health Act; Mansell 3 etc).
Appreciation of governance Desirable Participation in service developments Experience in delivery of quality initiatives.
