Job summary The post holder is required to work within a Multidisciplinary Team - with adults and children with a learning disability and associated complex health needs.The post holder will be responsible for taking on appropriate assessment of a persons needs and undertaking interventions that aim to improve their quality of life and achieve positive outcomes - -this will include planning how to do this alongside other professionals within the team.Post holder will provide education, knowledge, guidance and support to student nurses and unqualified staff in the team and provide support to clients and their families.

Health.-Awareness of current developments within a clinical setting Completed training in mentoring & Assessing student nurses Desirable Awareness of current developments within - Learning Disability services oUnderstanding of clinical governance Personal Qualities Essential Ability to communicate clearly and accurately Ability to adapt to new and changing circumstances Self-motivated Desirable Commitment to continuous improvements to clients care Skills Essential Ability to prioritise and work proactively Ability to manage a designated caseload without direct supervision Desirable IT skills OTHER RELEVANT REQUIREMENTS Essential Ability to work as part of a team Flexible approach to the needs of the service Lone worker Ability to travel between locations in a timely manner WELSH LANGUAGE REQUIREMENTS Desirable Welsh speaker desirable
