The service covers 7 days per week, 8am to 8pm.
The successful applicant will need to be flexible, have relevant experience, possess excellent communication skills, and actively contribute to team working.
You will be supported by the Senior Staff and the Team Manager.
We are seeking full-time applicants; however, part-time hours will also be considered.
A valid driving license and access to a vehicle for work purposes is required.
Open only to applicants from Hampshire & Isle of Wight NHS system.
Main duties, tasks & skills required To provide a high standard of clinical care, encompassing a holistic approach under the direction of a trained nurse, in patients' homes.
Working within a defined community nursing caseload for a group of GP Practices as part of the Integrated Community Care Team.
The post holder will maintain safe practice by following relevant Trust policies; holistic nursing assessments and care plans are recorded in line with Trust and NMC standards.
The post holder will work with a high degree of autonomy and professional accountability.
A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community.

Person specification Qualifications Essential NVQ3 or a commitment to undertake in-house work-based equivalent.
