\n Connect2socialwork are urgently looking for a Social Worker to join their great Mental Health team with a Pay rate up to £35 an hour on a locum Part Time basis.
\n The benefits of this Social Worker role: \n \n Hybrid working \n Supportive team to work in \n Laptop and training provided \n Low case load \n \n Job Description; \n \n Assess Client Needs: Evaluate clients' physical, emotional, and social needs through interviews, assessments, and observations.
\n Develop Care Plans: Create individualized care plans that address clients' specific needs and goals, and regularly review and adjust these plans as necessary.
\n Provide Support and Counseling: Offer emotional support, counseling, and guidance to clients and their families to help them cope with challenges and improve their well-being.
\n Coordinate Services: Work collaboratively with healthcare providers, community organizations, and other professionals to coordinate services and resources for clients.
\n Advocate for Clients: Advocate on behalf of clients to ensure they receive the necessary services and support, including housing, healthcare, and financial assistance.
\n Maintain Records: Keep accurate and up-to-date records of client interactions, assessments, and progress.
\n Crisis Intervention: Provide immediate support and intervention in crisis situations to ensure clients' safety and well-being.
