Main duties of the job Liaise with patients, carers, nursing staff, GPs and other health care professionals to obtain drug and allergy histories.Review prescriptions, reconcile medicines and identify and resolve pharmaceutical care issues in line with Departmental practice.Review prescriptions and provide clinical information on unlicensed medicines particularly in areas where information is lacking and medical or other opinion differs.Review patients' own medication to ensure safe and effective use.Optimise medication to ensure safe and effective treatment for patients and make appropriate recommendations.
