Job summaryWe are seeking to recruit a positive and self-motivatedindividual to take on the role of Clinical Pharmacist within our Pharmacy Team.The successful clinical pharmacist candidate will play an important role withinour pharmacy team and other members of the PCN multi-disciplinary team.
Main duties of the job Working as part of a multi-disciplinary team ina patient facing role to clinically assess and treat patients within yourcurrent competence, using your expert knowledge of medicines for specificdisease areas Being responsible for the care management ofpatients with chronic diseases and undertake structured medication reviews toproactively manage people with complex polypharmacy Providing specialist expertise in the use ofmedicines whilst helping to address both the public health and social careneeds of patients and to help in tackling inequalities Support the leadership team on ensuringperson-centred medicines optimisation and quality improvement is achieved,whilst contributing to the quality and outcomes framework and enhanced services Support patients to take their medications toget the best from them, reduce waste and promote self-care where possibleSupport the clinical aspects of shared careprotocols, clinical research with medicines, liaison with specialistpharmacists, liaison with community pharmacists and other healthcareprofessionalsBeing part of a professional clinical networkand have access to appropriate clinical supervision.Contribute to clinical governance activities /clinical audit to ensure continual improvement of service delivery.Provide services to support the practicesdeliver QOF, DES, DMOS and IIF targetsWork to agreed standards, monitor and maintainthe standard of care offered.

Being responsible for the care management of patients with chronic diseases and undertake structured medication reviews to proactively manage people with complex polypharmacy.
Providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities.
Support the leadership team on ensuring person-centred medicines optimisation and quality improvement is achieved, whilst contributing to the quality and outcomes framework and enhanced services.
Support patients to take their medications to get the best from them, reduce waste and promote self-care where possible.
Support the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and other healthcare professionals.
Being part of a professional clinical network and have access to appropriate clinical supervision.
Contribute to clinical governance activities / clinical audit to ensure continual improvement of service delivery.
Provide services to support the practices deliver QOF, DES, DMOS and IIF targets.
Work to agreed standards, monitor and maintain the standard of care offered.
Ensure you are competent and appropriately trained to carry out your areas of responsibility, including keeping up to date with practice policies and clinical standards Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
Support and participate in shared learning across the practice and wider organisation.
Ensure confidentiality is adhered to at all times.
Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
Use relevant IT systems to support role including SystemOne, TeamNet, Word, Excel, Access, and other systems where necessary.
In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
Be flexible in undertaking any other duties as deemed appropriate to the role.
Assist in promoting and maintaining your own and others health, safety and security as defined in the Practice Health & Safety Policy and the practice infection control policy and published procedures.
Prioritise own and others workload and ensure effective time-management strategies are embedded within the culture of the team.
Evaluate patients response to health care provision and the effectiveness of care.
Effectively manage own time, workload and resources.
To complete any required training or courses relevant to/required of your role.
