Job summary We have an exciting opportunity for an experienced Clinical Pharmacist to join ourexisting team within South East Shropshire Primary CareNetwork (SES PCN).
We are recruiting for 1 WTE Clinical Pharmacist who has ideally completed their CPPE pathway, and who is already a confident independent prescriber.The position of Primary Care Network Clinical Pharmacist has an essential role in improving the healthcare we provide to our patients.The successful candidate will be part of a dynamic and forward-thinking team of Clinical Pharmacists employed by Our Health Partnership, working across the practices within SES PCN to deliver consistency and excellence in medicines optimisation across the member practices.Professional Leadership will be provided by the Consultant Pharmacist at OHP and the Lead Clinical Pharmacists in the PCN.
Main duties of the job We are looking for pharmacists who will work as part of amulti-disciplinary team, developing and managing medicines optimisationservices within our practices.
Demonstrating improvements in value and patientoutcomes and contributing to reducing health inequalities through the safe andappropriate use of medicines.
This includes implementation of systematicprocesses for improving medicines optimisation across our PCNs and the widerOHP network.Work programmes will be prioritised according to the populationneeds of the PCN and in line with the PCN Directed Enhanced Service (DES).Please see attached Job Description for further information about thispost.

The practice lists vary from 2,300 to over 16,000 patients.
You will work across a selected group of practices.You will be employed on behalf of the PCN by Our Health Partnership (OHP).
OHP was set up by local GPs who arepassionate about providing high quality primary care and using their time andskills effectively to benefit patients.We are currently a GP partnership of 30 practices with 39sites, serving around 280,000 patients in Birmingham, Wolverhampton, andShropshire.The partnership offers a shared administrative andmanagement structure, cutting down the time doctors have to spend on admin.
Itopens up economies of scale to get best value from budgets and can access newfunding streams that are only available to large GP organisations.
It has theresources to develop innovative services and effective partnerships with localhospitals and care services.Please note, the Our Health Partnership Head Office is based in Cotteridge, Birmingham but this role will be based within any of the practices in the aforementioned PCN.
Job description Job responsibilities The post holder is a Clinical Pharmacist, who acts withintheir professional boundaries, supporting and working alongside a team ofHealth Care Practitioners within general practice and a Primary Care Network(PCN).The post holder will provide primary support to generalpractice staff with regards to prescription and medication queries.
They willperform medicines reconciliation on transfer of care and implement systems forsafer prescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patients in the PCN.The post holder will work as part of a multi-disciplinaryteam in a patient-facing role.
They will take responsibility for areas ofmedicines optimisation within the PCN and undertake structured clinicalmedication reviews to proactively manage patients with complex polypharmacy.The post holder will provide clinical leadership onmedicines optimisation and quality improvement (QI) and manage some aspects ofthe quality and outcomes framework and enhanced services at practice and PCNlevel.The post holder will work with all sectors of pharmacyacross the health system and the wider OHP organisation to improve populationhealth.
Demonstrating improvement in patient outcomes and working to reducehealth inequalities across the PCN contributing to improving the efficiency ofgeneral practice and delivering a best in class service.
Person Specification Qualifications Essential Undergraduate degree in pharmacy Registration with the General Pharmaceutical Council Independent prescribing qualification Completed CPPE Pathway Experience Essential Experience working as a Clinical Pharmacist.
Experience of working in Primary Care.
Experience of using IP qualification.
