Key Responsibilities: Deliver comprehensive clinical pharmacy services across a network of GP practices.
Collaborate with healthcare professionals to optimize medication use and improve patient outcomes.
Conduct medication reviews, provide prescribing guidance, and manage complex medication regimens.
Contribute to the development and implementation of clinical pathways and protocols.
Participate in audits, clinical governance activities, and service improvement initiatives.
Essential Requirements: GPhC registration Minimum 2 years of clinical pharmacy experience within a primary care setting.
Excellent communication, interpersonal, and organizational skills.
A strong commitment to patient-centered care and improving healthcare services.
