Job summary We are looking torecruit a suitably qualified and registered Clinical Pharmacist under EnglandsAdditional Roles Reimbursement Scheme (ARRS) to work in our expandingmultidisciplinary team (MDT) on behalf of Harthill Primary Network member practices,to be based primarily at Willerby & Swanland Surgery and Anlaby Surgery butwith a requirement to work on other practice sites as required.

This is a new anddeveloping role; you will hold an undergraduate degree in pharmacy, registrationwith the General Pharmaceutical Council (GPC) and be enrolled in, be willing tobe enrolled in, or qualified from an 18-month training pathway for Primary Careor equivalent.
You should have anIndependent Prescribing qualification.
Main duties of the job You will be workingalongside our general practice teams as part of our new and quickly developingARRS team across the PCN.
You will undertake a variety of tasks in support of thecare management of patients; processing of medication change requests viahospital discharge letters, reviewing patient and general medication queries inface to face clinics where appropriate, including NOMAD/dossette boxes, supportingwith memory aids for patients, and undertaking structured medication reviews,opiate and amber drug monitoring reviews.You will proactively manage people with complex polypharmacy, those withchronic diseases, the frail and patients with co-morbidities.
You will be involved with shared decisionmaking.
About us Harthill Primary CareNetwork (PCN) is a network of five NHS GP Practices within the Haltemprice andMarket Weighton areas of the East Riding of Yorkshire which includes practicesin Cottingham, Willerby, Anlaby, North Ferriby and Market Weighton.
We are one of seven PCNs across the EastRiding and cover a registered population of over 35,000 patients.Yorkshire HealthPartners Ltd (YHP) is managing the recruitment, employment and leadership ofpeople under NHS Englands Additional Roles Reimbursement Scheme (ARRS) forHarthill PCN.YHP is a GP ledorganisation established in 2012.
It currently has 22 member practices acrossthe East Riding and Hull with a combined list size in excess of 329,188 patients.It was initially established to support independent partner led generalpractice by holding contracts for primary care as well as focusing on theprinciple of care closer to home.
YHP seek to provide a strong voice forprimary care, patients, and staff, both as a separate entity, and inpartnership with others.
Job description Job responsibilities Job PurposeTowork as part of a multi-disciplinary team in a patient facing role toclinically assess and treat patients using your expert knowledge of medicinesfor specific disease areas.Job SummaryWorkingwithin your clinical competencies alongside general practice teams you willprescribe, be responsible for the care management of patients with chronicdiseases and undertake clinical medication reviews to proactively manage peoplewith complex polypharmacy.Key Areasof Responsibility To proactively manage people with complexpolypharmacy, especially the elderly, people in care homes, those withco-morbidities (in particularly frailty, COPD, and asthma) and people withlearning disabilities or autism (through STOMP Stop Over MedicationProgramme).
Deliver patientfacing/remote clinical medical reviews, including long-term conditions clinics andensure medicines optimisation.
Make appropriate recommendations to SeniorPharmacists, GPs or nurses for medicine improvement and monitoring.Work with care homestaff improving the safety of medicines ordering and administration.Attend and/or referpatients to multidisciplinary care conferences.Provide specialistexpertise in the use of medicines whilst helping to address both the public andsocial care needs of patients at our PCNs Practices.Help tackle health inequalities.
Provide training/developmentto primary healthcare teams on therapeutics and medicines optimisation.Work with generalpractice teams to ensure compliance with CQC standards.
OperationalManagementThrough structuredmedication reviews, support patients to take their medications to get the bestfrom them, reduce waste and promote self-care.Managecommon/minor/self-limiting ailments within the scope of your competence andpractice.
Provide patientfacing, remote, or telephone clinics for those with questions, queries, andconcerns.Manage medicines atdischarge from hospital, including identifying and rectifying unexplainedchanges working closely with the patient and community pharmacists so patientsreceive the medications they need post discharge.Signpost patients tothe appropriate healthcare professional for their care within an appropriate period, pathology results, common or minor ailments, acute conditions, long termcondition reviews etc.Produce and implementa repeat prescribing policy within each PCN practice.Manage the repeatprescribing reauthorisation process by reviewing patient requests and reviewingmedicines at review dates.
Ensure patients have appropriate monitoring tests inplace as required.Implement local andnational guidelines and formulary recommendations, monitoring practiceprescribing against the local health economies RAG list, making recommendationsfor GPs.
Assist practices in seeing and maintaining a practice formulary ontheir computer system and audit practice against the NICE technology assessmentguidance.
Work collaborativelywith all partners, including PCN MDTs, CCG Pharmacists and Technicians toensure consistent patient care, benefits and medicine optimisation.
FinancialManagement and Planning Contribute pharmaceutical advice for the developmentand implementation of new relevant services.
Analyse, interpret,and present medicines data to highlight issues and risks to support andinfluence decision making.Buildingcapacity through effective leadership Provide leadership on person-centre medicinesoptimisation including ensuring prescribers in the practices conserveantibiotics in line with local antimicrobial stewardship guidance and qualityimprovement.
Support furtherintegration of general practice with wider healthcare teams to improve patientsoutcomes, ensure better access and help manage general practice workload.
Managementof Communication Develop relationships and work closely with otherpharmacy professionals across the PCNs and wider health care system.
Answer relevantmedicine-related enquiries from GPs, other network staff, other healthcareteams and patients.
Suggest and recommend appropriate solutions and providefollow up for patients to monitor the effect of any changes.
Provide newslettersor bulletins on important prescribing messages.
Support public healthcampaigns.
RiskManagement and Quality AssuranceContribute to the Qualityand Outcomes Framework and Investment and Impact Fund criteria.
Take a central rolein the clinical aspects of shared care protocols, clinical research withmedicines, liaison with specialist pharmacists including mental health andreduction of antipsychotic use in people with learning disabilities, liaisonwith community pharmacists and anticoagulation.Be part of aprofessional clinical network and have access to appropriate clinicalsupervision to include one supervision session per month by a Senior Clinical Pharmacistand an assigned GP Clinical Supervisor for support and development.
Identify cohorts ofpatients at high risk of harm from medicines through pre-prepared practicesearches (patient related, medicine related or both).Review medicinesconnected to unplanned hospital admissions and readmissions via audit andindividual patient reviews.
Put in place changes to reduce the prescribing ofthese medicines to high-risk patient groups.
Set up and managesystems to ensure continuity of medicines supply to high-risk groups ofpatients.Undertake clinicalaudits of prescribing in areas as directed by the GPs, feedback, and implementchanges collaboratively.Implement changes tomedicines that result from MHRA alerts, product withdrawal and other nationalguidance.Managementof workload and personal development Act onown initiative in prioritising and managing workload, delegating tasksappropriately and referring to designated Board Members in respect of clinical,managerial, and administrative areas, as necessary.
