Main duties of the job The Clinical Pharmacist will be responsible for acombination of patient facing work such as medication reviews, chronic diseasemanagement, prescription queries and management of medicines through transferof care.
They will also play a significant role in service and qualityimprovement through audits of relevant work processes, sharing of best practiceand current guidance regarding prescribing and through work in medicinesoptimisation.The successful candidate will be supported in termsof clinical learning and professional development through regular access to aGP clinical supervisor, other clinical pharmacists and through a formaleducation pathway through the Centre of Postgraduate Pharmacy Education.
Aspart of a growing team of Clinical Pharmacists there is also a programme ofpeer support events and team meetings in place.

Our larger size allows for more collaborative working and due to the success of integrating the role into our practices, we are looking to grow our team and recruit additional Clinical Pharmacists to work alongside the current team.Chorley and South Ribble PCN is made up of the following practices: Buckshaw Village Surgery Adlington Medical Centre Lostock Village Surgery Medicare Unit Surgery Eaves Lane Surgery Croston Medical Centre Job description Job responsibilities Clinically assess and treat patients using expert knowledge of medicines for specific disease areas prescribing where appropriate.
Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, recommendation of medication changes and communication accordingly.
Support colleagues and patients with medication queries and monitoring..
Undertake clinical medication reviews to proactively manage people with complex polypharmacy.
Focus especially on those with multiple co-morbidities.
Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients.
Using audits and action planning to help to tackle inequalities.
Provide leadership on person-centred medicines optimisation.
Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services.
Use structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care.
Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components ( advice on treatment pathways and patient information leaflets).
Work with general practice teams to ensure that practices are compliant with CQC standards where medicines are involved.
Take a lead role in some aspects of supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists.
Person Specification Experience Essential Minimum 2 years experience as a pharmacist Experience of medication reviews relevant in General Practice Registration with General Pharmaceutical Council Desirable More than 2 years experience as a pharmacist Qualifications Essential Master of Pharmacy of equivalent Independent prescribing
