Job summary An opportunity has arisen for a full-time clinical pharmacist to join South Uttlesford PCN on a fixed one year contract, to provide high quality patient focused pharmacy services to the patients of South Uttlesford.
This role will be based across our four practices according to need, so the successful applicant will need to travel between sites.
Main duties of the job Patient facing long-term condition clinics to review the ongoing need for each medication.
Patient facing clinical medication reviews.
Patient facing care home medication reviews.
Patient facing and telephone medicines support.
Unplanned hospital admissions review of medications and reconciling of medications.
Repeat prescribing.
Identification of high risk patients.

Our PCN covers 4 practices across west Essex: Peacock Surgery, Angel Lane Surgery, John Tasker House supporting 54,000 patients.

Person Specification Qualifications Essential Degree in Pharmacy (Masters or equivalent), be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society.
Ideally you will be a non-medical independent prescriber but the PCN will actively support you to achieve this, if not already an independent prescriber.
Experience Essential You will be required to lead by example, setting and maintaining high standards of professionalism within a team environment.
You should be IT literate and confident in using practice systems.
