University College London Hospitals NHS Foundation TrustWe have an exciting opportunity to recruit a Band 6 Clinical Nurse Specialist in the Asthma Service.
This is for a one-year fixed-term contract.
The role will involve supporting the asthma service with a focus on introducing an app-based tool to assist patients with asthma management.
The post holder will support the wider asthma service in various ways, including providing rapid access clinics, case finding, pre-assessment telephone clinics, face-to-face clinics, and community liaison between primary and secondary care.
There will be an educational component to the role, primarily for internal staff but also for local community referrers.
Additionally, there will be audit and research opportunities.
The successful candidate will have experience in asthma, either in a ward, community, or outpatient setting, with at least two years of post-registration experience.
They must be able to practice autonomously and be self-directed while also operating efficiently as part of a team.
An eagerness to learn and develop, along with a passion for education, is essential.
The candidate will have the opportunity to contribute to the development of the service.
Main Duties of the JobThe postholder will work closely with the lead clinician, Dr. Roy, and a Band 7 Asthma Nurse Specialist, supported by the Lead Respiratory Nurse Specialist.
This role provides an excellent opportunity to develop leadership skills related to change management and service development.
Case finding within the trust and the community via MDTs and diagnostic hubs.Support asthma management by reviewing patients using the digital asthma app and other clinical information to triage referrals effectively.Pre-clinic assessment to support one-stop clinics and straight-to-test opportunities.Deliver patient-centered care by acting as a point of contact for patients and GPs, ensuring a community link with primary care services.Support patient-initiated follow-up (PIFU), educating patients on self-management and when to trigger PIFU.Education and training to promote strategies for long-term and acute management plans associated with asthma across the community and the trust.Undertake patient assessment using various methods, which may include physical examination, ordering and interpreting diagnostic tests, or advanced health needs assessment.Utilize a diverse range of knowledge in decision-making to determine evidence-based therapeutic interventions.About UsUniversity College London Hospitals NHS Foundation Trust (UCLH) is one of the most complex NHS trusts in the UK, serving a large and diverse population.

Salary: £44,806 to £53,134 a year, per annum inclusive of HCAS.
