Job summary The post-holder is responsible for Trust-wide effective and time efficient patient flow management at admission and or admission avoidance through same day emergency care.
Working closely with medical consultants, Clinical Directorates and other departments.
The main focus will be to work alongside the medical registrar and on-call team of the day to review new admissions and outliers in wards.
Ensuring the patient has an appropriate pathway and that discharge is planned from admission.
Excellent communication skills are required with the ability to build strong professional relationships with ED, Radiology, Pathology, diagnostic departments and the Site Team.
To ensure pathways for admission avoidance are utilised fully, triaging GP referrals, and utilising medical same day emergency care.
Ensuring that all new medical patients are on the medical list, and seen in the post take ward round at the first possible opportunity.
Main duties of the job The successful post holder will have excellent communication skills.
Working with the acute physicians, the on-call medical consultant and the medical registrar to optimise the acute take and maintain safety.
To act as a self-directed practitioner who directs, provides and evaluates patient care, founded on agreed management care plans and protocols To work independently and without direct supervision in a variety of clinical settings, Trust-wide and in the community, prioritising workload to meet the regular unpredictable situations that may occur within this role.
To demonstrate the ability to work independently and perform comprehensive patient assessments based upon history taking and physical examination.
To demonstrate highly developed clinical reasoning and decision-making.
As appropriate, to order diagnostics, interpret results, advise on treatments and/or refer to relevant health care professional To have the necessary knowledge, skills and training to recognise emergency clinical situations such as anaphylaxis, cardiac and respiratory deterioration, to act appropriately, knowing when to seek assistance To be a part of the decision-making within ED and the wards and be responsible for the safe discharge of some very high risk patients, ensuring that the appropriate care is in place, often organising a rapid response from within the Hospital at Home Team About us We are committed to being an inclusive and diverse employer.
