Job summary Are you an experienced Registered Mental Health Nurse and Independent Prescriber, looking for a new and rewarding challenge in crisis and urgent care services?Our Home Treatment Team in Bridgwater is on the lookout for a Clinical Lead to join our friendly team.
You will act as a key member of our dynamic, multi-disciplinary team, responsible for ensuring urgent community assessments of clients presenting in an acute mental health crisis.You should possess strong leadership and clinical decision-making abilities, enabling you to effectively support and guide the team in collaboration with the Team Manager.
Your expertise in prescribing will be essential for ensuring the safe and effective management of medications.
Additionally, you will bring solid assessment skills, experience in risk management, and the ability to make dynamic decisions.
A genuine passion for delivering compassionate care to clients, as well as their families and carers, is essential for this role.We would be willing to consider a developmental post for the right candidate.
You would start on a Band 6 and will be funded to complete the Physical Assessment and Clinical Reason Course and Independent Prescribing Course at University of West successful completion you will progress to a Band 7 Clinical Lead within the team.
Main duties of the job We recognise that recovery from acute mental illness is best met within a persons own home, so we strive to provide safe, intensive mental health treatment and intervention at home, as an alternative to admission to psychiatric hospitalYou will be a key member of a skilled Multi Disciplinary Team who will offer a wide range of psychological, medical and social interventions to support recoveryYou will work as the Clinical Lead within the team,supporting the team with case discussions and formulation, risk management and decision makingAs well as working with a clinical caseload and their families, the home treatment team gatekeep admissions to, and facilitate early discharge from mental health inpatient settingsThis role will involve flexible working across a 7 day a week service About us At Somerset NHS Foundation Trust, we're committed to supporting our employees with a range of benefits designed to enhance your professional and personal life.

They must be flexible, creative and responsive to managing the changing needs of the service which will be challenging at timesPlease see full Job Description for detailed information Person Specification Qualifications Essential RMN Current registration with Nursing and Midwifery Council Evidence of Post qualifying professional development in an acute setting Desirable Act as an independent prescriber, ensuring safe and effective medication management Experience Essential Significant post registration experience in multi-disciplinary community mental health services Experience of managing risk of clients in crisis Experience of assessment, care-planning, development and evaluation in conjunction with clients and carers Experience of providing clinical leadership within a team Good knowledge of resources and working practices across health and social care Ability to be innovative in practice Desirable Experience of Independent Prescribing Additional Criteria Essential Able to carry out evidence based interventions Exceptional interpersonal skills with the ability to communicate effectively and empathically with patients, carers and relatives about complex and sensitive issues Excellent organisational skills, ability to manage own time and plan timed activities for staff and patients Ability to recognise and manage challenging situations in a calm and professional manner Ability to lead a team and work effectively as part of a team Experience of carrying out supervision to junior staff, coordinating the care of clients and delegating tasks Ability to inspire hope, support recovery and make a difference Act in a ways that support equality and diversity The ability to attend the necessary training with regard to personal safety Ability to work under pressure and on own initiative Flexibility and commitment to high standards of service for adults with mental health difficulties Ability to speak to distressed patients and their relatives in person and on the telephone Required to spend a considerable period of a working day in front of a VDU with expected high standards of written communication skills Ability to travel round Trust area to fulfil the requirements of the role Ability to work in partnership with other agencies
