Job summary We are lookingfor an occupational therapist, physiotherapist, nurse or social worker who hasexperience of working in or with community care equipment and / or wheelchairservices, to join the ICB working across Hull and East Riding with the ClinicalCommissioning Lead to support and deliver integrated Community Services, withthe specific remit for Community Care Equipment / Wheelchair Services andPersonal Wheelchair Budgets.
The post will work across Hull and East Riding Places, Hull CityCouncil and East Riding of Yorkshire Council.
We arelooking to recruit a candidate who possesses highly developed and effectivecommunication and organisational skills, with knowledge and experience ofequipment and wheelchair services, combined with initiative and drive to workflexibly to meet the needs of the service.Your key areasof focus will include the ongoing review of clinical pathways, prescribing /authorisation policies and processes.
It will include making efficiencyrecommendations and implementing approved service improvements which lead toimproved outcomes for service users, their carers and/or families.As anexperienced clinician you will provide leadership with key partners,clinicians, service users and their carers/families in order to implement thedelivery, monitoring and reviewing of agreed plans for the ongoingtransformation of Community Care Equipment and Wheelchair Services.
Main duties of the job Keyfunctions of the role are to support the continued provision of community careequipment and integrated wheelchair services including on-going development ofeligibility criteria to ensure the service is delivered to meet identifiedoutcomes.Youwill act as a Community Care Equipment and Wheelchair Services authoriser onbehalf of the joint commissioners.
This involves providing authorisation forspecial requests, premium service levels ( same day/next day delivery) ornon-contract activity on a daily basis.You will leadthe on-going review of community care equipment to address product andoperational/service performance issues including reviewing and making requiredchanges to standard stock catalogues to ensure effective and efficient servicedelivery.
Monitor the outcomes of any such reviews/changes and leadrequirements for further service development and improvements identified.

Job description Job responsibilities Key functionsThe Clinical Commissioning Delivery Manager: will be required to support the Lead Commissioner for the Community Equipment and Wheelchair Service, to: Contribute and support the continued provision of community care equipment and integrated wheelchair services including on-going development of eligibility criteria to ensure the service is delivered to meet identified outcomes.
This will be achieved through the evaluation of a range of alternative solutions.
Project manage and provide leadership to the development of integrated models of care/pathways across Health and Social Care, to ensure service users have timely access to a range of standard and specialised community care equipment and wheelchairs, that meet their personalised and identified assessed needs.
Provide appropriate clinical challenge regarding community equipment and wheelchair requests acting as a Community Care Equipment and Wheelchair Services authoriser on behalf of the joint commissioners.
This involves providing authorisation for special requests, premium service levels ( same day/next day delivery) or non-contract activity on a daily basis.
Develop training packages to both support and develop the implementation of new models of care / pathways.
Deliver the training to both, health providers and other health and social care organisations.
Work with relevant business intelligence and finance colleagues to ensure that business and financial administrative systems are in place and operating effectively on behalf of all partner agencies.
Monitor and maintain a robust management process for the numerous peripheral stores across Hull and the East Riding of Yorkshire.
Lead the on-going review of community care equipment to address product and operational/service performance issues including reviewing and making required changes to standard stock catalogues to ensure effective and efficient service delivery.
Monitor the outcomes of any such reviews/changes and lead requirements for further service development and improvements identified.
Support the robust authorisation process to ensure limited resources are best deployed to meet the highest level of need.
This involves acting as the accountable officer to approve nonstock (special items).
Represent and/or lead on a clinical basis as required at defined groups, meetings and forums, for example, the Adults and Childrens operational meetings.
Responsible for ensuring service users and carers engagement in the ongoing review of the community equipment and wheelchair services.
Responsible for establishing positive relationships with all levels of clinical and non-clinical staff.
Responsible for the production of accurate and relevant statistical reports to support plans and recommendations and presentation at Committees and Boards as required.
Lead on delegated projects across the ICB and Local Authorities as required.
Ensure that processes for the management of projects adhere to governance rules, particularly in terms of procurement and conflict of interest.
Contribute to the ongoing delivery of Personal Wheelchair Budgets/Personal Health Budgets in line with NHS Frameworks Person Specification Qualifications Essential Relevant degree and current clinical professional registration (Nurse, Occupational Therapist, Social Worker, Physiotherapists etc.)
Evidence of continuing professional development Specialist knowledge regarding community equipment and/or wheelchairs Desirable Post Graduate/Masters qualification in relevant field Awareness of personal health budgets.

Experience of translating legislation/policy/standards and guidance into practice Experience of building and sustaining effective and productive working relationships with a wide range of people at all levels and across a wide variety of agencies Experience of public and patient involvement in service planning Desirable Management experience in different health/social care settings Operational management experience
