Job summary A new role has been developed to support the co-ordination of wards within elderly and acute general medicine at Pinderfields Hospital.The successful applicant needs to be dynamic and pro-active supporting the medical and nursing team in delivering a high standard of compassionate patient centred care.You will be an important part of the clinical and non-clinical MDT so need to have excellent communication skills and working well within a team is essential to excel at the role.
Main duties of the job Working under the direction of a registrant, participate fully in the delivery of planned nursing task to aid a safe and timely discharge, within the boundaries of your role which may include identifying bloods to be taken, cannulate patients requiring diagnostic investigations,Ability to provide essential patient care to patients when required.Track discharge medications supporting timely discharges.Support discharge planning during ward board rounds.To work with:the nurse in charge to complete delegated tasks which ensure that patients are discharged home in a timely manner.the nursing team to ensure that all educational needs have been met prior to the patient being discharged,the nursing teams and allied health professionals to ensure a seamless discharge between the hospital and the community is maintained.MDT, clinical site manager, clinical flow team and flow matron to ensure flow is maintained through the ward at all times.Assist the Nurse in Charge with delegated tasks.Support in the review of stranded and super stranded patients.Maintain accurate records of patient flow through the unit.Demonstrates own duties to new starters and supervises work of students and HealthCare Support WorkersActs on own initiative when delivering patient care with supervision accessible.To work alongside the clinical team supporting patients on the ward to be nursed in an appropriate ward/area.

This includesbut not limited to, ordering equipment , catheter supplies, nephrostomysupplies, district nurse requests, outpatient appointments , TWOC clinic.To work with allied health professionals to ensurethat patients are discharged home safely and in a timely manner with thecorrect support that is required on discharge , equipment, rehabilitation,care package.To ensure that bed spaces are fully cleaned andprepared for the next patient.
Assist the Nurse in Charge with delegated tasks,supporting with the flow of patients, including dealing with telephoneenquiries contacting patients families etc.
whilst maintaining confidentialityat all times.Support in the review of stranded and super strandedpatients.Maintain accurate records of patient flow throughthe unit.Demonstrates own duties to new startersand supervises work of studentsand HealthCare Support WorkersFrequent exposureto emotional/distressing events,, breaking of bad news.Participate in patient and public involvement activities.Undertake mandatorytraining and role specific trainingrelevant to role.Acts on own initiative when delivering patientcare with supervision accessible.To work alongside the clinical team supportingpatients on the ward to be nursed in an appropriate ward/area.

Can competently perform a range of the core skills listed Desirable Previous experience of complex health needs Experience Essential Experience of working in a Health and Social Care setting in a Band 2 role or equivalent Desirable Experience with NHS digital systems Qualifications Essential Must have: Level 2 in numeracy and literacy (or equivalent) Completed Care Certificate accredited by e-learning for healthcare evidence of both theory and all practical elements of the skills for health 15 standards (training completed via e-learning only will not be accepted.
Plus, must have one of the following: Vocational Level 3 qualification in a Health-related subject regulated by OFQUAL.
