Job description Job responsibilities PurposeTo assist in the delivery of care activities in the child/young persons own home and in the community following an intensive training programme and competency assessment.
Staff will work unsupported following assessment with the support of Clinical Case Manager.
Participates in the induction and development of new staff, under the direction of the Clinical Case Manager.
To be flexible to work unsocial hours, including night duty over a 24 hour period, 4/5 week rota pattern including weekends and Bank Holidays Principal duties to include:Communicationskills (factor 1)Establishes and maintains effective verbal andwritten communication with colleagues, children/young people, families andmembers of the multidisciplinary team, providing clinical information asrequired.

Person Specification Qualifications Essential Good general education and literate in English and numerate Level 2 NVQ or equivalent in health related subject Desirable NNEB NVQ level 3 in Childcare, Care or equivalent working with children Experience Essential Experience of working with children or caring for children in a health, education or social setting.
Understanding of a range of work procedures and practices, some of which are non-routine, which require a base level of theoretical knowledge.
Desirable Good understanding of child development Good understanding of how chronic illness affects the family.
