Job summary The Stoke on Trent 0-19 Integrated Health Visitor and School Nurse Service has a vacancy for a dynamic, energetic and forward thinking Band 4 Children, Young People and Families Practitioner working 30 hours per week based at Smallthorne Health Centre.The successful candidate will demonstrate commitment, drive and enthusiasm and work towards the service development and trust values.

You must have a Nursery Nurse Qualification or equivalent for this role (see Job Description).The Stoke on Trent 0-19 Integrated Health Visitor and School Nurse Service offers an integrated, flexible and family focused service providing high quality, compassionate personalised support and care to Children and Young People across Stoke-on-Trent.
You will be part of a team delivering the 0-19 Healthy Child Programme for a varied, unique and diverse population, in supportive skill mix teams, working closely with our partner agencies across health, social care and the local authority.We are committed to supporting personal development of our staff and have excellent training opportunities.
The successful applicant must have the ability to travel within the Trust.
Main duties of the job The 0-19 Children's, Young People and Families Practitioner will work within uni/multi-disciplinary teams in addressing a range of care and support issues across a spectrum of needs.
They will contribute to the delivery of the Healthy Child Programme (0-19) and to the improvement of the health and wellbeing of the population served.
The nature of the service will be determined by local service 0-19 Children, Young Peoples and Families Practitioner will work in a variety of settings including clinics, Family Hubs and schools, providing advice, information, guidance, signposting and referrals.
