Job summary CHAWPCN have an exciting opportunity for an experienced Clinical Pharmacist to join our PCN which coverspractices across Chelford, Handforth, Alderley Edge, Wilmslow (CHAW) PrimaryCare Network.We are looking for anexperienced Clinical Pharmacists who ideally is a prescriber and has completedtheir CPPE course to join our current team of 6 pharmacists and 2 pharmacytechnicians.
The role is a full-timerole, working out of the PCN Hub located in Wilmslow Health Centre, face toface consultations in practice and very limited work from home.
Main duties of the job The successful candidatewill work across the 5 GP Practices in the PCN and will cover a range ofpharmacy activity including clinical letters, patient queries, structure medicationreviews, general medication reviews and hypertension clinics.

Job description Job responsibilities Job Purpose: The post holder is a pharmacist, who acts within theirprofessionalboundaries, supporting and working alongside a team ofpharmacists ingeneral practice.
In this role they will be supportedby a senior clinicalpharmacist who will develop, manage and mentor them.The post holder will work as part of amulti-disciplinary team in a patient-facingrole.
The post holder will take responsibility forareas of chronic diseasemanagement within the practice and undertake clinicalmedication reviews toproactively manage patients with complex polypharmacy.The post holder will provide primary support togeneral practice staff withregards to prescription and medication queries.
Theywill help support therepeat prescription system, deal with acuteprescription requests, andmedicinesreconciliation on transfer of care and systems for safer prescribing,providingexpertise in clinical medicines advice while addressing both publicand socialcare needs of patient in the GP practice (s).The postholder will ensure that the practice integrates with community andhospitalpharmacy to help utilise skill mix, improve patient outcomes, ensurebetter accessto healthcare and help manage workload.
The role is pivotal toimproving thequality of care and operational efficiencies, so requiresmotivation andpassion to deliver an excellent service within general practice.Key duties and responsibilities Patient facing Long term condition clinicsSee (where appropriate) patients with single or multiple medicalproblems where medicine optimisation is required ( COPD, asthma).Review the on-going need for each medicine, a review of monitoringneeds and an opportunity to support patients with their medicines takingensuring they get the best use of their medicines ( medicinesoptimisation).
Make appropriate recommendations to Senior Pharmacists or GPs formedicine improvement.Patient facing Clinical Medication ReviewUndertake clinical medication reviews with patients and producerecommendations for senior clinical pharmacist, nurses and/or GP on prescribingand monitoring.Patient facing care home medication reviewsUndertake clinical medication reviews with patients and producerecommendations for the senior clinical pharmacist, nurses or GPs onprescribing and monitoring.
Work with care home staff to improve safety ofmedicines ordering and administration.Management of common/minor/self limiting ailmentsManaging caseload of patients with common/minor/self-limitingailments while working within a scope of practice and limits of competence.Signposting to community pharmacy and referring to GPs or otherhealthcare professionals where appropriatePatient facing medicines support Provide patient facing clinics for those with questions, queriesand concerns about their medicines in the practiceTelephone medicines support Provide a telephone help line for patients with questions, queriesand concerns about their medicines.Medicine information to practice staff and patientsAnswers relevant medicine-related enquiries from GPs, otherpractice staff, other healthcare teams ( community pharmacy) and patientswith queries about medicines.Suggesting and recommending solutions.Providing follow up for patients to monitor the effect of anychangesUnplanned hospital admissionsReview the use of medicines most commonly associated withunplanned hospital admissions and readmissions through audit and individualpatient reviews.Put in place changes to reduce the prescribing of these medicinesto highrisk patient groups.Management of medicines at discharge from hospitalTo reconcile medicines following discharge from hospitals,intermediate care and into care homes, including identifying and rectifyingunexplained changes and working with patients and community pharmacists toensure patients receive the medicines they need post discharge.Set up and manage systems to ensure continuity of medicines supplyto highrisk groups of patients ( those with medicine compliance aids orthose in care homes).Signposting Ensure that patients are referred to the appropriate healthcareprofessional for the appropriate level of care within an appropriate period oftime pathology results, common/minor ailments, acute conditions, long termcondition reviews etc.Repeat prescribingProduce and implement a practice repeat prescribing policy.Manage the repeat prescribing reauthorisation process by reviewingpatient requests for repeat prescriptions and reviewing medicines reachingreview dates and flagging up those needing a review.Ensure patients have appropriate monitoring tests in place whenrequired.Risk stratificationIdentification of cohorts of patients at high risk of harm frommedicines through pre-prepared practice computer searches.
This might includerisks that are patient related, medicine related, or both.Service developmentContribute pharmaceutical advice for the development andimplementation of new services that have medicinal components ( advice ontreatment pathways and patient information leaflets).Information management Analyse, interpret and present medicines data to highlight issuesand risks to support decision making.Medicines quality improvementUndertake clinical audits of prescribing in areas directed by theGPs, feedback the results and implement changes in conjunction with thepractice team.Medicines safety Implement changes to medicines that result from MHRA alerts,product withdrawal and other local and national guidance.Implementation of local and national guidelines and formularyrecommendationsMonitor practice prescribing against the local health economysRAG list and make recommendations to GPs for medicines that should beprescribed by hospital doctors (red drugs) or subject to shared care (amberdrugs).Assist practices in seeing and maintaining a practice formularythat is hosted on the practices computer system.Auditing practices compliance against NICE technology assessmentguidance.Provide newsletters or bulletins on important prescribingmessages.
