We have an exciting opportunity for a Carer Support Adviser to join our team in East Lincolnshire.
As a Carer Support Adviser, working 32 hours per week, you will be part of our Operational Team, helping to improve the lives of carers through your work.
This role will include: Managing conversations with carers to identify need; Writing statutory carers assessments on behalf of Lincolnshire County Council; Providing information, advice, and wellbeing support; Running wellbeing groups; Pro-active identification of carers through partnership working.
To be successful, you will have experience of working with adults either in a 1:1 or group setting, a working knowledge of health and social care particularly in relation to adults, and excellent communication and interpersonal skills.
A high standard of written skills is needed in order to help the carer identify their support needs and to complete statutory assessments.
