Amica Care Trust offers a range of friendly, welcoming and professional support services, including occasional day care, to assisted living providing the independence many of our resident's desire, through to specialist dementia care for those in most need of our support.
We are looking for a Care Transition Specialist to join a fantastic and supportive team.
This is a full-time position, 37.5 hours per week, Monday to Friday with occasional weekends.
Day to Day: Conduct thorough pre-admission assessments of potential residents to determine their suitability for admission liaising with the Home Manager on decisions for a placement.
Develop and complete individualised care plans for new residents, ensuring all their needs, wishes and preference are met in a person-centered way.
Based on the residents' needs and wishes, identify any potential training required to support their care needs.
Work with other professionals on transition plans when required to ensure a clear structured process is in place to reduce distress to residents being admitted to the home.
Ensure that any input, advice or requirements are implemented on admission (where appropriate) from other services such as the Multi-Disciplinary Team including but not limited to; Speech and Language Therapists, Occupational Requirements: You will need to be a Registered Nurse with an active PIN for this role.
You would have had previous experience working as a nurse and experience in caring for residents with different health needs, including advanced dementia.
You will have experience in safeguarding and clinical risk assessments and have a strong understanding of Care transitions – admission and discharge processes You will have previous experience in a care home or similar healthcare setting You will be expected to work within our staff values and promote these at all times by being an effective communicator, passionate about your work, encouraging team working and having a positive attitude.
