Job summary Rethink Mental Illness is seeking a committed Care Navigator to provide tailored, high-quality support to individuals being released from HMP Millsike.
As part of our integrated mental health team, youll work closely with healthcare and prison staff to ensure that health services are in place to support a smooth, positive transition from prison to community life.
Using a strengths-based, coaching approach, youll offer one-to-one and group interventions, helping individuals manage their own wellbeing and reconnect with essential community resources.
Main duties of the job Key Responsibilities Deliver pre- and post-release support to individuals with health vulnerabilities, assisting with continuity of care and connecting them to community services.
Conduct initial assessments and care mapping for service users, creating tailored care plans for effective release and reintegration.
Organise and contribute to health promotion events, supporting the reduction of health inequalities and the promotion of wellbeing.
Maintain accurate, detailed records in accordance with organisational and national standards, and manage multiple tasks to meet the demands of a high-volume, fast-paced environment.Qualifications & Skills NVQ Level 3 or equivalent in Health and Social Care or related field.
Experience supporting vulnerable individuals to set and achieve goals, with knowledge of health, criminal justice, housing, and social care.
Strong communication skills with experience in case management, structured interventions, and care planning.
Proficiency in Microsoft Word, Excel, and database systems, along with effective time management and problem-solving abilities.Desirable:Experience within a prison setting or working with individuals involved in the criminal justice system, and additional mental health or coaching training.

Person Specification Experience Essential I am educated to NVQ Level 3 or equivalent with qualifications or training relevant to the Care Navigation role, Health and Social Care I have relevant experience of supporting vulnerable individuals to identify, set and meet their own goals using 1:1 and/or group interventions I have a working knowledge of health, debts, social care, housing, court systems, mental health, criminal justice and substance misuses I have experience of working to support individuals using structured, time limited support and am confident with this way of working I am passionate and motivated to support individuals leaving the criminal justice system through provision of a high quality, professional level of care, recognizing and respecting individual differences and promoting diversity I value the unique life journey each individual has been on, and embrace the challenges this can bring I am patient, calm and respectful within my work I hold a full UK driving license and have access to my own transport for work purposes I am committed to developing my knowledge and skills in coaching, and am prepared to complete training within the role to support this I am experienced in maintaining professional boundaries, even in challenging situations I have the ability to make a comprehensive assessment of an individuals situation and needs; to develop, implement and review risk management plans and devise support plans to meet these needs.
I have excellent time management skills, understanding the need to work effectively and efficiently at all times I have the ability to develop professional relationships with patients and colleagues, and practice in a non-discriminatory manner, respecting the beliefs and cultural practices of individuals and groups.
I have experience of using clinical outcome measures to inform practice.
I am a confident communicator with excellent verbal and written skills.
I have working knowledge of standard IT packages including Microsoft Word, Excel and use of database / information systems.
I welcome ongoing feedback to identify areas for personal growth and development, and strive to continually improve my practice I have the ability to use clinical supervision and personal development positively and be self-reflective in own personal and professional development.
I am able to prioritise workload demands and positively respond to unforeseen challenges, seeking guidance and clarity from managers as appropriate.
I deal with stressful and difficult situations in a calm manner and seek guidance from managers to inform my practice.
Desirable Experience of working in prison environment or experience of working with offenders.
Experience and / or training in coaching Specialist interest and/or training in working with older adults, veterans, substance misuse, probation systems, youth offending or females who have been affected by the criminal justice system Mental Health qualifications or knowledge, including therapeutic interventions
