Job summary Role: Care Navigator Oakfield Surgery, NewmarketWe are a group of 7 GP practices across Suffolk and weare seeking to recruit an enthusiastic and flexible Care Navigator, with strongcustomer service skills, for our Oakfield Surgery site in Newmarket.Thevacancy is for between 20 - hours a week; however, this may be negotiable forthe right candidate.
Flexibility is crucial to accommodate training, holidaysand occasional sickness.Oakfield Surgery supports in excess of 7,500 patientsfrom a diverse range of backgrounds.
We have an established team with a strongfocus on customer service and communication.
We would love to hear from candidateswith GP surgery experience.
We are also keen to hear from those that can showflexibility and a range of transferable skills.The successful candidate should be able to workcalmly and professionally as part of a team.
You should have the ability tomulti-task.
A strong command of written and spoken English with an excellenttelephone manner is a must.
Good time management skills and the ability tofollow protocols and or instruction and attention to detail is essential.

Main duties of the job To provide comprehensiveadministrative support for the general practice and to act as focal point forpatients and staff.To receive and assist and directpatients in accessing the appropriate service or healthcare professional in acourteous, efficient and effective way.Offer general assistance to thepractice team and project a positive and friendly image to patients and othervisitors either in person or via the telephone.Facilitate effectivecommunication between patients and members of the healthcare community.
Handle patient complaints inaccordance with practice protocols.To provide an efficient andaccurate support to the secretarial team, clinicians & patients.

Ensure an effective and efficient receptionservice is provided to patients and any other visitors to the Practice Deal with all general enquiries, explainprocedures and make follow-up appointments as requested by clinicians.
Using your own judgment and communication skillsensure that patients with no prior appointment but who need urgent consultationare seen in a logical and non-disruptive manner.
Explain Practice arrangements and formalrequirements to new patients and those seeking temporary cover and ensureprocedures are completed.
Process personal and telephone requests forappointments, visits and telephone consultations, and ensuring callers aredirected to the appropriate healthcare professional.
Register requests for home visits, ensuring carefulrecording of all relevant details and where necessary refer to the relevantClinician.
Divert calls and take messages, ensuringaccuracy of detail and prompt appropriate delivery.
Advise patients of relevant charges for privateservices.
Enter patient information onto the computer asrequired.
Ensure that all new patients are registered ontothe computer system promptly and accurately Patientnotes and correspondence: Cover colleagues holiday/sickness periods asrequestedPremises: oOpen up premises at the start of the day whenfirst to arrive and make all necessary preparations to receive patients.
o When last to leave at the end of theday, ensure that the building is totally secured.
Undertake any other additional dutiesappropriate to the post as requested by the POM, which could include clinicalcoding.
