The Role We are seeking an experienced Care Manager to join our dedicated team and help shape the future of community-based care.
In this professional and varied role, you will assume responsibility for a caseload of clients with complex care requirements, ensuring that each individual's needs are assessed, understood, and met with diligence, respect, and empathy.
Your key responsibilities will include: Care Planning and Review: Assessing new clients and regularly updating existing care plans to maintain high standards of person-centred support, identifying any changes in health or personal circumstances and adjusting the care plan accordingly.
Client and Carer Support: Acting as the main point of contact for both clients and carers maintaining open lines of communication to foster trust and confidence ensuring clients feel listened to and carers feel supported and valued.
Supervision and Quality Assurance: Providing guidance, mentorship, and supervision to the carers allocated to your clients.
Auditing daily documentation to ensure compliance with regulatory standards and internal quality benchmarks Safeguarding and Professional Liaison: Handling more complex issues including safeguarding referrals.
Working with external professionals such as multidisciplinary teams and social workers to achieve the best outcomes for your clients.
Regular Client Visits: Conducting home visits to assess the ongoing suitability of care plans, verifying that your clients' environments remain safe.
Offering reassurance to clients and their families that they are receiving the highest standard of care Requirements 2-5 years of management experience in a social care, healthcare, nursing, or social work setting RMN/RGN registration or a Level 5 Diploma in Health and Social Care (or equivalent) strongly preferred Proven experience managing complex caseloads and supervising staff Exceptional written and verbal communication skills, with the ability to liaise effectively with clients, carers, and external professionals Full UK Driving License and access to own vehicle essential Full right to work in the UK (sponsorships not offered with this position) Strong organisational, time management, and problem-solving skills Working knowledge of best practices, employment regulations, and safeguarding procedures Meticulous attention to detail and the ability to work under pressure in a fast-paced environment Collaborative team player with a flexible, adaptable approach to an ever-changing industry Compassionate, person-centred mindset with a genuine passion for enabling clients to live independently Benefits Competitive Salary of £28,000 - £35,000 per annum, depending on experience BUPA Health Plan 32 Days Paid Annual Leave (Including Bank Holidays) Loyalty Annual Leave (up to 5 additional days) Government-Backed Pension Scheme Employee Assistance Programme Comprehensive Ongoing Training and Professional Development Hybrid working pattern with required office days dependent on location Excellent opportunities for career progression and internal promotion At ENA Care Group, we're on a mission to transform the lives of our clients through exceptional, person-centred care.
