Job summary An exciting opportunity has arisen for a Care Home Nurse atNunwell Surgery in Bromyard.We are seeking applicants who are clinically experienced,motivated, enthusiastic, and able to work autonomously and safely within thelimits of their own competence and enjoy working collaboratively within a team.
The roles main focus will be to provide high quality, planned and proactive care for care home patients and to avoid unnecessaryhospital admissions.
Main duties of the job The Care HomeNurse will maintain a high profile professional leadership role across the Practiceinclusive of clinical governance and a commitment to high-quality, evidencedbased practice.
They will ensure thatthe Enhanced Care in Care Homes Framework and Primary Care Network (PCN)Enhanced Health in Care Homes Directed Enhanced Service (DES) is translatedinto the provision of evidenced based practice within Care Homes and scope ofown practice.
Care Homes within thePractice catchment area include Nursing and Residential.The Care Home Nursewill build on and strengthen the link between all health and care professionalsdelivering care and support to care home residents inclusive of colleagueswithin General Practice and Community Services, Social Care, Care Home Teams,and the Voluntary Care Sector.
The Nurse willwork collaboratively and proactively within a multi-disciplinary team (MDT) toensure care and support for care home residents is seamless and timely.
Therole is not expected to receive referrals for same day urgent care as it isproactive rather than reactive in nature, however there will be a need torespond within an MDT approach to coordinate ongoing care resulting from achange in patient condition.
About us Situated in a delightful market town with excellent transport links toHereford, Worcester and Malvern, a supportive ethos runs throughout thepractice and helps us maintain our established and loyal team.We are a GMS practice with 9500 patients.
The team comprises 6 partners and3 salaried GPs, an excellent nursing team including two nurse practitioners,two practice nurses and 2 skilled health care assistants, trusted andhardworking reception and admin teams working alongside our busydispensing staff and practice pharmacists.
Job description Job responsibilities Working within the Care Home Team to provide patient-focused care.
To support the practice with ongoing management of care home patients by reviewing patients and amending Personalised Care & Support Plans and RESPECT forms.
To support care home patients with anticipatory care discussions and planning.
To provide acute medical treatment for care home patients with undifferentiated and undiagnosed problems seeking a primary health care service including, telephone triage, face to face consultations, and home visits.
To assess and manage patients with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate.
Independently prescribe (where applicable to the role) and medication reviews, review appropriate medications in accordance with local policies and evidence based medical practice and work collaboratively with the wider team to support patients and to promote medication adherence.
To order investigations, follow-up results and arrange management including seeking advice where appropriate.
To work with a multidisciplinary team including community and voluntary sector to improve care home patient support.
Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
Attend ward rounds and MDTs.
A prescribing qualification for this post is essential and an interest in frailty would be beneficial.
