Job summary We are looking for a Receptionist/Care-coordinator to work 16 hrs pw between 8:00am - 7:00pm.
An excitingopportunity has arisen for an experienced, enthusiastic, motivated andhard-working individual to join our busy practice to provide an efficient andfriendly service for all our patients and visitors, and to provideeffective administrative support to our doctors and members of the clinicalteam.We are a friendly, dynamic and forward thinking practice.As part of the role, the Care Coordinator will: to utilise population health data to proactively identify patients;support patients to prepare for a shared decision-making conversation;holistically bring together identified care and support needs, within a single personalised care and support plan (PCSP);help patients manage their needs through answering queries, making and managing appointments, and ensuring that good quality written or verbal information is available to help with choices about care;support patients to take up training and employment, and appropriate benefits;assist with access to self-management education and support for patients to take more control of their health/wellbeing;assist patients to access personal health budgets where appropriate;provide coordination and navigation for people and their carers across health and care services, working closely other primary care professionals; andsupport the coordination and delivery of MDTs within the PCN.
Main duties of the job Duties will include: The ability to communicate with a wide range of people in a kind, professional and caring way.Work effectively with colleagues as part of a team.A willingness to learn and progress with a can do attitude.Effective IT skills are an advantage.While knowledge of GP practice systems is desirable, training will be available.work with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where appropriate, refer people back to other health professionals within the PCN;raise awareness of shared decision-making and decision support tools; andraise awareness of how to identify patients who may benefit from shared decision making and support shared decision-making conversations.Dealing with appointment, home visit and telephone requestsBooking, amending, and cancelling patient appointmentsProcessing of repeat prescriptions in accordance to practice guidelinesInitial contact with and responding to requests from patients, other team members and associated healthcare agencies and providersFiling and retrieving paperworkTaking messages and passing on informationKeeping the reception area tidy and free from obstructions and clutterEnsure patient confidentialityComplete annual mandatory training About us We are a small friendly, forward-looking practice with a practicepopulation of approximately 5000 patients.Our team comprise 2 Partners, 3 Locum GP, 1 Practice Nurses and 1clinical pharmacist, together with a strong and hard-working Administrativeteam.We are an accredited training practice and teach and support Medicalstudents at different levels.Schedule: Monday to Friday No WeekendsExperience: Receptionist: 1 year (preferred)Work remotely: No Job description Job responsibilities The purposeof the Receptionist's job is to ensure that patients are able to access healthservices including contact/make anappointment to see a doctor in the appropriate manner depending on need andthat all patient records are up-to-date and available for all users whenrequired.
The purposeof the job is also to contribute to maintaining practice administrative systemssuch as data input, correspondence andrepeat prescribing systems.
