Thisinvolves being responsible for assessment, developing, implementing andevaluating care plans using a person-centred approach.
Ourideal candidate would be a committed team player as well as being able to workindependently demonstrating initiative and passionate in promoting good qualitycare to maximise independence.
Within this role you will gain training,support, and guidance to ensure you are working collaboratively with externalagencies to give the best possible care for our service users.
Main duties of the job This rolerequires someone who can manage in a fast paced environment completingassessments to support diagnosis.
The day to day work for this role will be ledby the needs of the service users and the service which will require effectivetime management and prioritisation skills as well as effective communicationand autonomous working.
Allof our staff are an integral part of the multi-disciplinary team within mental health services.
The successfulcandidate will need to understand the importance of engaging with externalagencies within our area in order for our service users to achieve high-qualitycare that we in Navigo always strive to deliver.
