Job summary Joining our small team of Care Co-ordinators, the successful applicant will help West Meon Surgery deliver a first class health service to our patients.
We are looking for a team member who is passionate about the NHS and keen to support the Partners and clinicians in delivering the best quality accessible healthcare to all our patients.West Meon Surgery is a rural practice with a small practice population so we know all our patients well.
They expect the highest service from us and we are delighted to be able to meet their health needs effectively and efficiently.Rurality means that public transport connections are limited so team members will need to be able to drive to get to us.
Main duties of the job The Care Co-ordinator will help us arrange and deliver healthcare to our patient population.
Each member of the team has an area they specialise in.
We are looking for someone to take on managing our health campaigns - looking at innovative ways to engage patients to ensure that annual reviews are completed, that their medical record is accurate and concise and that each patient receives the best health care.We are looking for someone who can do 20 hours per week spread over three days (Monday, Tuesday and Wednesday).
Cover to is required on Wednesday.

We have a team meeting or training each Wednesday where everyone at work attends, with a staff quiz once a month just for fun.
Job description Job responsibilities TheCare Coordinator will be enrolled in, undertaking or qualified from appropriatetraining as set out by the Personalised Care Institute and works closely and inpartnership with the Social Prescribing Link Worker, social prescribing serviceprovider and Health and Wellbeing CoachTheCare Coordinator will receive, assist, and direct patients in accessing theappropriate service or healthcare professional in a courteous, efficient andeffective way.Tosupport the practices delivery of excellent healthcare by supporting annualrecalls and health campaigns, ensuring that patients feel a co-ordinatedapproach to their healthcare provided by West Meon Surgery.Job ResponsibilitiesIncrease take-up of cancer screeninginitiatives including bowel, breast and cervical, and improve the quality andtimeliness of referrals under the two week wait.Improve patient engagement with annualhealth checks and condition specific follow up within primary care.Help to raise awareness of health andwell-being, screening, annual health checks and reviews for long termconditions and how it can be promoted.Provideco-ordination and navigation through the health and care systems.Makereferrals to services and other health and care professionals.Helppatients prepare for/follow-up.Signpostpatients to informationWorkin partnership with MDT colleagues including social prescribing link worker(s)and health and wellbeing coach(es)Undertakescanning of clinical documents (such as clinic reports, 111 reports, and out ofhours information) are filed onto the Docman system and coded accordingly.Ensurean effective and efficient reception service is provided to patients and anyother visitors to the Practice.Dealwith all general enquiries, explain procedures and make new and follow-upappointments.Receiveand make telephone calls as required.Divert calls and take messages, ensuring accuracy of detail and promptappropriate delivery.Enterrequests for appointments or home visits on to the system, ensuring carefulrecording of all relevant details and where necessary refer to Duty Doctor.Advisepatients of relevant charges for private (non-General Medical Services)services, accept payment and issue receipts for same.Ensurethat all new patients are registered onto the computer system promptly andaccurately.Utilise population health intelligence to proactively identify andwork with a cohort of patients to deliver personalised careSupport patients to utilise decision aids in preparation for ashared decision-making conversationHolistically bring together all of a patients identified care andsupport needs, and explore options to meet these within a single personalisedcare and support plan (PCSP), in line with PCSP best practice, based on whatmatters to the personHelp patients to manage their needs through answering queries,making and managing appointments, and ensuring that patients have good qualitywritten or verbal information to help them make choices about their care, usingtools to understand patients level of knowledge, confidence in skills inmanaging their own healthSupport patients to take up training and employment, and to accessappropriate benefits where eligible for example, through referral to socialprescribing link workersAssist patients to access self-management education courses, peersupport or interventions that support them to take more control of their healthand wellbeingExplore and assist patients to access personal health budgetswhere appropriate;Provide coordination and navigation for patients and their carersacross health and care services, working closely with social prescribing linkworkers, health and wellbeing coaches, and other primary care professionals;andOther job responsibilitiesWork with the GPs and other primary care professionals within thePCN to identify and manage a caseload of patients, and where required and asappropriate, refer patients back to other health professionals within the PCNRaise awareness within the PCN of shared decision-making anddecision support toolsRaise awareness of how to identify patients who may benefit fromshared decision making and support PCN staff and patients to be more preparedto have shared decision-making conversationsHave basic safeguarding processes in place for vulnerableindividualsProvide opportunities for the patient to develop friendships and asense of belonging, as well as to build knowledge, skills and confidence.Discuss patient related concerns and be supported to followappropriate safeguarding procedures ( abuse, domestic violence and supportwith mental health) with a relevant GP.
