Job summary This exciting new role isfunded by the Grand Union Primary Care Network, as part of expanding GP teamsnationally.
The successful candidate will be part of a large dynamic teamworking in a 35,000 patient GP practice based over 3 sites.
Main duties of the job We are looking for individuals who will work with our team of GPs, nurses, pharmacists and admin staff to co-ordinate annual health checks for chronic disease and long term conditions.
You will be working directly with patients to offer support at home and identify health care needs through a holistic assessment.
