The CDSC will assistthe Primary Care Network (PCN) in delivering improvements to the services weprovide our patients in relation to the following aspects of patient care.
Main duties of the job Participation in national cancer screeningprogrammesEnsuring robust and supportive referral practicesare in place for patients suspected of having cancer; including use ofguidelines, professional development, and safety netting of those referred.Working collaboratively to shorten timescales ofdiagnostic intervals.In support of collaborative working the post holderwill form productive and supportive relationships with practiced staff who havethe skills, knowledge and remit to contribute to this work eg GP practicenon-clinical cancer champions, social prescribers and secretaries.The post holder will assist practices to evaluatetheir screening uptake and engage hard to reach populations to participate toreduce health inequalities.
This willinclude working alongside practices to enhance processes to track and follow-upscreening non-responders.Provide advice and support to practices on canceraudit/referral review of cancer diagnoses.To work with practices to collage the learning from case reviews toidentify any trends or learning.Develop with core staff across the PCN consistentsafety netting approaches/systems to monitoring patients who have been referredurgently with suspected cancer or for further investigations to exclude thepossibility of cancer.See the attached job description for full details.
