Job summary One of the NHS Long Term Plan ambitions is to improve the provisionof crisis assessment and brief response within the Emergency Department (ED)and in community settings.
Offering Children and Young People (CYP) assessment,alongside Intensive Home Treatment services for those who might otherwiserequire inpatient care.
Herefordshire and Worcestershire Health & Care NHS Trust aredeveloping a consistent home treatment provision, across Herefordshire.
Thisteam operates 7 days a week between the hours of 8am and 8pm.
The teams will offer an Intensive HomeTreatment service aimed at CYP who might otherwise require inpatient care, orintensive support that exceeds the normal capability of a generic children andyoung peoples mental health community team.
Main duties of the job The role willbe a little unpredictable, as you can be called at any time during a workingday to support in a crisis.
We will complete comprehensive risk assessments forevery CYP; these can be completed independently although will often discuss patientsthat are more complex as a Multi-disciplinary team.
The team works hard toprovide a fully integrated service for our CYP, so it is not just about makingsure their needs are catered for while they are in crisis, but also within thewider system.
We talk to families and carers, GPs, community mental healthteams, social care teams and voluntary organisations, and ensure the mostappropriate follow up support is in place, be this through an intensive hometreatment package from the team, or Core CAMHS or through the voluntary sector.Researchindicates that a more timely response to treating YP regarding their mentalhealth leads to them being less likely to require long term mental healthsupport, so this is an amazing opportunity to support the YP and their familieswithin Herefordshire.

Person Specification Experience Essential Extensive post registration experience in mental health care including mental health assessment and risk assessment Use of therapeutic interventions within mental health settings 1:1, counselling, family therapy and group work Delivering teaching / training Providing consultation Desirable Experience of working in a CYP service or Crisis & Home Treatment service Formulating and communicating a clinical presentation to an MDT Qualifications Essential Professional clinical qualification RMN / OT / Social Worker etc.
Evidence of post registered clinical qualifications/training or be able to demonstrate an equivalent level of training to a minimum of post graduate diploma level Current registration with relevant professional body I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Desirable Evidence of on-going continuing professional development Evidence of providing a supervisor role Skills & Abilities Essential Ability to communicate clearly and effectively in English in both verbal and written formats.
