Main duties of the job To provide a counselling service to patients, theirfamilies/carers and significant others both pre and post bereavement regardlessof cause.
The service offers counsellingservices to those who have a life limiting illness, those who are dying andthose who will be or are bereaved.
The successful candidate will be required to support theteam in providing one on one sessions and also support the team by facilitatinggroup work as required.The post is 8 hours per week but can be worked flexibly over the month to meet client need.
About us Hospiceat Homes core service is its skilled team of Registered Nurses and HealthcareAssistants who provide exceptional care and support in peoples last year oflife and at end of life, also providing additional services such asLymphoedema, Bereavement and Family Support, Befriending, Occupational Therapyand Complementary Therapy.Weprovide individualised care and support to improve quality of life and our endof life care helps facilitate a peaceful and dignified death in the home andsupport to those who are bereaved.Ourclinical service area covers a mixture of urban and rural communities, a regionof approximately 1,500 square miles throughout North and East Cumbria.
Job description Job responsibilities Responsibility for Patients/Families/Carers/Significant others Responsible for assessment of individuals presenting with multi, complex and sensitive issues with regard to counselling pre and post-bereavement.
Responsible for the development of therapeutic packages designed to meet individual need which respects patient autonomy and privacy at all times, assessing specific need in psychological, spiritual, religious and cultural domains.
Responsibility for the delivery of therapeutic packages within the individuals home, community hospital, GP surgery or other agreed venues.
Responsible for the evaluation of therapeutic packages in conjunction with the individual.
Responsible for recognising the need for intervention by other services psychology and referring to those services appropriately.
Provide sensitive and effective advocacy for the individual.
Responsibility for Training and Education Contribute to the service volunteer supervision/training programme through identification of topics for inclusion.
Provide information, advice and support to Health and Social Care Professionals in relation to bereavement issues.
Responsibility for Resources Prioritise and manage own caseload and practice on a daily basis.
Responsible for maintaining own caseload records in accordance with Hospice at Home and the British Association of Counselling and Psychotherapy (BACP) policies and procedures.
Identify resources required to carry out the role.
Responsible for the submission of mileage and timesheets on a monthly basis.
Responsible for inputting data regarding service provision on EMIS (computerised shared system).
Responsibility for Service and Self-Development Practice in accordance with Hospice at Home and BACP policies and procedures.
Provide service information for audit purposes.
Responsible for maintaining supportive network for self and arranging own clinical supervision in line with set minimum standards.
Identify personal training requirements.
Participate in annual appraisal process.
Person Specification Experience Essential Experience of working within palliative care Experience of working with a diverse range of individuals with multi and complex needs.
Experience of working and liaising with other agencies Qualifications Essential Degree or diploma in Counselling Registration with Professional Body
