Main duties of the job -To assess plan, implement and evaluate patient centered nursing care as per Hospice policies and protocols; - Actively work in partnership with patients and their families and carers; - Ensure patient, family and carers are central to care delivery and decision making; -To uphold and promote patient safety at all times; - To promote and support Advance Care Planning; - To seek feedback from patients, families and carers and share with the Incare leadership team where indicated; - To communicate effectively with patients, families and carers, the MDT and all staff within the Hospice to promote effective teamwork; - To deliver concise and accurate patient presentation in handovers and to the MDT meetings; - Communicate with other professionals outside the Hospice to facilitate continuity of care; - Ensure accurate and timely records of care (electronic and paper) are maintained, upholding information governance; - Promote evidence based practice through membership of clinical groups; - Actively promote hand hygiene and infection control policies to minimise infection risk; - Participate in audit and evaluation of clinical practice in the interests of promoting quality care; - Actively promote reflective practice; and - Actively promote an environment which enables the nursing team to work effectively.

We also offer outreach services and run a successful Lymphoedema clinic for patients.
