Main duties of the job Provide a clinical pharmacy service to thegastroenterology ward on a rotational basis.
Support the provision and development of a holistic, specialist pharmaceutical care service to patients receiving high-risk, high-cost medication ( advanced therapies, hepatitis anti-virals) delivered via Homecare services.
Support services provided to outpatients andendoscopy and provide a clinical pharmacy service to the daycase unit.Provide a ward-based clinical pharmacy serviceaccording to Trust agreed standards including: individual prescription reviewto optimise therapy, confirmation of the patients medication history, adviceon dosage, side-effects, cautions and monitoring required and offer duties.
Participate in ward rounds, clinical meetings ward user group orout-patient clinics as appropriate in order to provide pharmaceutical advice toprescribers and other health care professionals.

Person Specification Qualifications Essential Qualified to Masters Degree level (MPharm or equivalent).
Member of General Pharmaceutical Council (GPhC).
Post Graduate Diploma/MSc in Clinical Pharmacy or equivalent.
Experience Essential Clinical experience within gastroenterology/hepatology is Highly Desirable.
