Job Overview: We are seeking a dedicated and skilled Band 6 Physiotherapist to join our team, focusing on supporting patient flow and providing high-quality care in a fast-paced environment.
The role involves conducting same-day assessments and facilitating timely discharges within the Frailty Same Day Emergency Care (SDEC) setting.
You will play a crucial role in enabling rehabilitation and re-enablement to ensure effective discharge planning and improved patient outcomes.
Key Responsibilities: Perform comprehensive physiotherapy assessments for patients within the Frailty SDEC unit.
Plan and deliver tailored rehabilitation and re-enablement programs to meet individual patient needs.
Facilitate same-day discharges by coordinating with multi-disciplinary teams and implementing effective discharge plans.
Support patient flow by ensuring timely and appropriate interventions that align with care pathways.
Provide expert advice to patients, families, and caregivers to optimize recovery and promote independence.
Maintain accurate clinical documentation and contribute to the development of service improvement initiatives.
Requirements: HCPC registration as a Physiotherapist.
Proven experience in acute or frailty care settings, with a focus on rehabilitation and discharge planning.
Strong clinical assessment and intervention skills.
Excellent communication and collaboration abilities to work effectively within a multi-disciplinary team.
Ability to manage a dynamic caseload and adapt to a fast-paced environment.
