\n Job Overview \n This role is vital in supporting patient flow within the Frailty Same Day Emergency Care (FSDEC) unit.
Responsibilities include same-day assessments, rehabilitation, re-enablement, and discharge planning to ensure seamless care for patients with complex needs.
\n Key Responsibilities \n \n Perform same-day assessments and facilitate timely discharges within FSDEC.
\n Develop and execute individualized rehabilitation and re-enablement strategies.
\n Collaborate with multidisciplinary teams to optimize patient outcomes.
\n Conduct effective discharge planning to ensure continuity of care.
\n Maintain accurate and high-quality documentation per NHS standards.
\n \n Requirements \n \n \n HCPC Registration as a Physiotherapist.
\n Demonstrated experience in patient flow, rehabilitation, and discharge planning.
