Job summary Berkshire Healthcare NHS Trust is a Community and MentalHealth trust, providing a wide range of services to people of all ages livingin Berkshire.
And to do this, they employ approximately 5,000 staff who operatefrom over 60 sites across Berkshire, as well as out in peoples homes and invarious community settings.
Main duties of the job Assess and provide high standards of initial assessmentinput to patients in the community.
Liaise with MDT members in treatment and management care plansand working closely Support OT /PT with the recommendations for patientcentred approach to achieve their rehab goals Create, evaluate all care plans implemented and shouldhave a sound knowledge in discharge planning and ability to make furtherreferrals in to the community as required.

Person Specification Qualifications Essential Diploma / Degree in Physiotherapy / Occupational therapy.
HCPC Registration.
Travel Essential Should be able to drive and own a car for work purposes with business insurance.
Experience Essential 2-3 years UK NHS Experience.
