Job Overview: Join the Community Rapid Response Team to provide urgent, patient-centered occupational therapy services.
This role involves swift assessments and interventions aimed at preventing hospital admissions and supporting timely discharges.
Key Responsibilities: Conduct urgent assessments for patients in community settings.
Develop and deliver individualized treatment plans to promote independence.
Work closely with the multidisciplinary team to coordinate care.
Support patients with complex needs in preventing hospital admissions.
Key Requirements: HCPC registration as an Occupational Therapist.
Minimum of 6 months NHS experience in community or rapid response settings.
Strong clinical decision-making and communication skills.
Ability to work independently in a fast-paced environment.
