Job overview An exciting opportunity has arisen within the Adult Community Bladder & Bowel Service, for a band 6 Specialist Bladder and Bowel Nurse.
You will be working alongside a highly skilled specialist nursing team including a Bladder and Bowel Service Clinical Lead, Specialist Bladder and Bowel Nurses and Assistant Practitioner.
We are looking for a highly motivated Nurse who has a passion for caring for patients with bladder, bowel and pelvic floor dysfunction.The successful candidate will work in partnership with our patients and their families/carers to lead in the delivery of person centred, safe and effective care.We will provide a training package to ensure that you are fully equipped to manage patients with a bladder or bowel dysfunction.You must be able to provide recent and ongoing clinical professional development, and be able to work autonomously as well as part of a team.Travel is an essential part of this role please demonstrate on your application form how you would meet this requirement.
Main duties of the job To work according to the MCHFT Trust' Value and Behaviours', supporting members of the Bladder and Bowel team to achieve the service specific and organisational objectives· To provide a continual pathway of support for patients cared for within Central Cheshire Integrated Care Partnership (CCICP) Community.· To provide Level 1 fundamentals of bladder and bowel care and training to staff employed within CCICP and care home staff.· To support prevention of harm to patients with bladder and bowel dysfunction and reduce risks associated with these conditions.· To increase quality of life for the patients and reduce stigma related to Bladder and Bowel dysfunction.· To assess, plan, implement and evaluate individualised/person centred care in the most efficient and clinically effective way whilst maintaining the patient's privacy, dignity and confidentiality.· To provide advice and support to junior staff in the delivery of high-quality care and actively support the Community Adult SPN and Clinical Team Lead in the delivery of training and care within CCICP and Care homes in the CCICP footprint.· To support and action service improvement plans as delegated by the Head of Nursing, Clinical Lead and their Specialist Nurses.

Detailed job description and main responsibilities To promote fundamentals of bladder and bowel care and prevention of risk associated to bladder and bowel dysfunction using evidence-based practice.To provide evidence-based Level 1 bladder and bowel training to CCICP and care home staff to help promote safe and effective care.Act as a role model for excellence in the delivery of bladder and bowel care and the MCHFT Trust 'Values and Behaviours'Work within and promote CCICP/MCHFT policy and proceduresTo actively engage in delivering and identifying areas for service improvementDeputise for the clinical lead as requiredTo support the clinical Lead and Bladder and Bowel team in the day-to-day operational functioning of the service.Be responsible for the assessment, planning, treatment, implementation and evaluation of person centred care for designated patients.Ensure that the management of verbal and physical aggression, sexually diverse behaviour and vulnerability are sensitively handled in line with policy and procedure, and that the individual's dignity and privacy is always maintained.In the absence of the Adult Bladder and Bowel Specialist Nurse is responsible for ensuring continuity of care in accordance with the care programme, seeking support from other specialist practitioners/operational managers as and when required.To provide Education and Health Promotion to prevent, improve and manage bladder and bowel dysfunction and symptoms to patients and health care workers both face to face and by telephone/remote consultation.To provide a pathway of support for patients with Bladder and Bowel dysfunction to move between different health care and community settings.
Liaise with multi-disciplinary agencies to support the ongoing management of patients and their bladder and bowel conditions.
