Job summary Anexciting full time/part time opportunity has arisen to recruit RegisteredNurses to work at Darlington Memorial Hospital or University Hospital of NorthDurham within the Discharge Lounge.
We strive to provide an effective andefficient service to our patients who require our input in assisting with theirdischarge Discharge Lounge provides a warm caring environment in whichpatients can safely wait for discharge home or transfer to an alternativesetting such as community hospital within the Trust.
Excellentcommunication skills are essential to ensure a safe discharge and an increasein patient flow around the hospital.
The postholder will be required tocommunicate with the Wards, Doctors, Pharmacy, Relatives, Carers, and Transportand Patient Flow Team.
The postholder will supervise discharge lounge HealthCare Assistants, liaise with the Discharge management team/ward coordinators andrelatives.
The postholder will be responsible for patient care during theirtime in the discharge lounge and efficient coordination of booking patienttransport and checking prescribed medication in readiness for safe discharge.

Thepost holder will have a nominated base and be expected to travel and workacross both sites, if required, to meet service need during times of pressure.
