Band 4 Social Care Co-ordinator is required for a large public sector organisation based through the Belfast Trust Location: Shankill Wellbeing Centre Pay Per Hour: £12.86 Part Time hours: 37.5 hrs JOB SUMMARY/MAIN PURPOSE: The social care co-ordinator will work alongside the social worker on a designated caseload for which the social worker holds professional responsibility.
Under the direction and guidance of the social worker, the social care co-ordinator will work to ensure the implementation of person centred interventions, activities and or services to support and meet assessed need and to achieve desired outcomes, as well as contribute to the monitoring of the wellbeing of service users in accordance with the social work intervention plan.
The social care co-ordinator will be delegated by the social worker, specific low risk, non complex case work, where the focus is on prevention, confidence building, monitoring and enabling the older person to engage with and gain support from community and voluntary services.
The social care co-ordinator will actively seek to establish an empowering relationship with service users and their carers and at all times through their actions demonstrate dignity, compassion and respect.
He /she will be expected to develop their knowledge base around the aging process and issues that affect older adults and their carers, and will be strong advocates who champion the rights of Older People.
The social care co-ordinator will be expected to register with the Northern Ireland Social Care Council and adhere to the standards of conduct and practice MAIN DUTIES / RESPONSIBILITIES To work with and under the direction of the social worker to deliver a person centred social care service to Older People, their families and carers as part of a delegated caseload.
To carry out a range of activities and interventions in accordance with individual care plans in order to maximise service user's potential, promote community integration, reduce isolation, promote confidence and build individual resilience to promote and maintain wellbeing To contribute to the monitoring of the effectiveness of domiciliary care packages and other services in place to support service users to live in their own home, through a range of agreed monitoring and observation activities to include home visits, discussions with service user and family carers, and liaising with community and voluntary service providers and relevant others Work to build a knowledge base of local community and voluntary networks and encourage and assist service users to make use of these resources to enhance wellbeing and quality of life To liaise effectively with key staff from commissioned services in respect of appropriate care and support commissioned on behalf of service users, in order to seek and provide assurances that care packages are being delivered to the standard and quality required, are meeting the assessed need and that resources are being used efficiently and effectively.
When this is not the case, or if there is a change in the service users need or ability, report same promptly to the social worker When appropriate to do so, liaise with the multi-disciplinary team including district nurse, GP's and hospital staff, regarding the needs of service users Under the direction of the social worker, promote self Directed Support as an alternative to directly provided services Use information obtained through a range of monitoring activities to inform the professional review, which is led and co-ordinated by the social worker in accordance with the care plan and assessed need Support responsible risk taking and contribute to the identification and management of risk by participating in regular case discussions with the social worker, taking into consideration individual service users and carers expressed views, aspirations, goals and choices obtained through the working relationship Be alert to and report promptly indicators of changing need in order to prevent unnecessary deterioration or risk of harm Contribute as appropriate to the implementation of protection plans and attend strategy meetings in line with adult safeguarding procedures Throughout all aspects of the role, promote the Human Rights of service users To support the social worker to comply with internal administrative processes around the commissioning and monitoring of packages of care as agreed To contribute to the effectiveness of the team through supporting effective communication systems, attendance at meetings, and contributing to and adhering to team processes To contribute to the team duty system in accordance with team procedures ESSENTIAL CRITERIA Five GCSE's including English Language (Grades A-C) or equivalent OR NVQ Level 3 in Care or equivalent AND A minimum of 2 years paid or voluntary experience working with Older People and or people with physical health and disabilities in either a social care or hospital setting in a caring capacity Either be in a position to register with the NISCC or already be registered.
