Job summary To assess, provide and review Nursing care, advice and information to patients and carers within the home environment.
Aim to wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.
To take part in the daily rota, to lead the UCR Team and co-ordinate care, and have clinical oversight of the caseload and work under the direction of the UCR Manager (using their skills and knowledge as a Registered Nurse).

You wont be disappointed.
The Honiton and Ottery St Mary (HOSM) Urgent Care Response Team (UCR) are looking for an experienced Nurse to join them.
The successful candidate will be expected to collaborate with the Multi-disciplinary team members using your clinical assessment skills to avoid unnecessary Hospital admissions and focus on patient flow with discharges from the acute Hospital.
You will ensure high standards of Nursing care are delivered and continuously review plans for patients on the caseload.
You will be a pivotal part of the Integrated Health and Social Care Team, and with excellent communication skills you will liaise with the patient on admission to caseload about discharge planning.
You will work in conjunction with Occupational Therapists and Physiotherapists to promote patient independence and safety at home.
The team is skilled mixed with registrants and non-registrants and you will provide training and support to develop the team to deliver excellence.
You will be provided with regular supervision and support with ongoing professional development as identified in your annual appraisal.
Clinical skills for the role include Advanced Clinical Assessment Skills, Catheterisation, Venepuncture, Wound Care, End of Life care, IV therapies, assessing and ordering of equipment, attending daily meetings regarding patient flow and to have knowledge of CHC fast track applications, and Social Care Reablement referrals.
