The Community Matrons and Care-Coordinators are a forward-thinking and innovative service that is community based.
The team works in a fully integrated way, with physiotherapists, occupational therapists, district nurses and community matrons all working side by side to ensure an integrated and holistic approach to the care of people accessing our service.
Main duties of the job You will be expected to carry out clinical visits to patients in their own homes and residential homes under the direction of the registered nurse.The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal and IT skills and be able to communicate effectively with patients, carers and other members of the MDT.We are interested in hearing from both Assistant Practitioners and Nurse Associate's with proven clinical and communication skills to work as part of our friendly, supportive and innovative team.

All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.
As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.
The induction will be held at our head office in Rainham, Essex.Probationary PeriodThis post will be subject to a probationary period.
