Main duties of the job Working in a busy and dynamic Adult Community Mental Health Team means every day will be different.
You will be involved in: Managing a (capped) caseload with a mix of service users, from those under The Care Programme Approach and others that are notUndertaking joint initial urgent/routine assessments on behalf of the serviceAttending multi-disciplinary team meetings with our Consultant Psychiatrists, Nurse Consultants, Clinical Team Managers, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologists, Clinical Associate Psychologists and Assistant PsychologistsAttending outpatient clinic appointmentsUndertaking face to face visits in the community setting including home visitsWorking with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholdersMedication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication managementWorking with other teams services like; inpatient, Home Treatment Team, Psychiatric Liaison Team, Social Care, The Police, Accommodation and Support Teams and 3rd party sector.We work with service users on their presenting needs and have many evidence based intervention packages to enhance the Service User experience.Use of own car and ability to travel independently is essential About us Why work for us?
