Assistant Care Co-Ordinator For Cambridge

Job summary We are recruiting for a full time ( hours per week) assistant care- co-ordinator to support care provision and patient flow in our Intermediate Care Service.You will work across the county wide Intermediate Care Service supporting colleagues based in our South teams primarily.
You will be able to work flexible shifts patterns within our core service hours of 7am to 10pm, 7 days a week.
Shifts in early and late patterns will be required to cover service 7am - 10pm, including weekends and bank holidaysThe vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital as part of the discharge to assess model.
Main duties of the job The job role is to assist the Care Co-ordinator within the Intermediate Care Service who will supervise and support the post holder to ensure referrals are managed in a timely and safe manner The job is focused on receiving new referrals into the relevant Intermediate Care Service, and assisting to co-ordinate and organise packages of care delivered by the local Immediate Care Service to support patients living in the community.

Please confirm in your application that you meet the specified criteria.Please refer to the attached job description and person specification for full details of responsibilities To communicate with both external health care professional and referrers, private care agencies, local authority staff and services Senior Care Coordinator to seek clarification and communicate the requirements and arrangements for all patients referred to the local Intermediate Care Service.Identify the need for and ensure the timely onward transfer of information for referrals who require comprehensive needs assessments by other members of the Intermediate Care Service.Ensure that necessary patient information supporting the monitoring of all intermediate care referrals is recorded in an accurate, consistent, and timely manner.Maintain daily data bases of all new, current and patients waiting Intermediate Care Services including the current pathway the patient is on, date of entry to pathway, allocated intermediate care staff, date of review, date of discharge and services required on discharge.Assist with monitoring the progress of referrals and ensure any subsequent reviews are initiated and completed.Support the reporting requirements of the service as requested by the care co-ordinator and team leader.Ensure any data that is used and managed with line with Trust Information Governance and Information Security policies.Where required, to cover face to face care calls with patients.To attend patients homes to complete staff supervision.To provide cover across service operation hours from 7am-10pm including weekends and bank holidays.

*DVLA have a number of reciprocal arrangements with overseas countries, for further information please visit the DVLA website Person Specification Education / Qualification Essential Evidence of a good standard of general education/GCSE or equivalent Evidence of ability to study Willingness to undertake training to meet competency framework Evidence of relevant short course and in-service training NVQ Level 3 or equivalent Desirable To have completed recent care certificate or be committed to complete on employment Knowledge of specific patient conditions, types of therapy intervention acquired through training and experience Experience Essential Experience of undertaking of administration work in a community setting Desirable Experience in staff development, such as assessments or practice supervision Related experience of working in a rehabilitation/Reablement or healthcare environment with older people Experience in line management of staff.
Skills and Abilities Essential Satisfactory level of IT skills Standard keyboard skills and ability to communicate through IT using packages such as Word, Office Ability to organise groups of carers to deliver care in patients' homes and deal with unpredictable situations Excellent communication skills Ability to work well autonomously as well as part of a team Desirable Knowledge of CPFT and local health and social care services Experience of working with SystmOne Knowledge & Understanding Essential Good knowledge of Health and Safety legislation and safe working practices Accepts responsibility for own workload Recognises the limits of own authority within the role
